 MD Writes Order for Blood Product & Obtains Consent 
  1. Consent must be obtained by MRP


 Nurse to ensure Crossmatch & Consent has been Obtained
  1. One Consent needed per hospital admission
  2. Crossmatch is valid for 96 hours from sample collection
Prepare for Blood transfusion
  1. Ensure IV access (Initiate/Flush IV)
  2. Obtain vital signs/lung sounds
  3. Prep IV Tubing with IV Pump 
Obtain Blood Product from Lab
  1. Lab to call floor when blood product ready
  2. Bring Downtime BBK Issue Voucher BW to Lab and extra labels for any blood product derivatives (albumin, RhIg, IVIG)
  3. Only have 20 minutes to start blood transfusion from the time the blood product is removed from the lab fridge
Double Check Blood Product with Second Nurse at Patient’s Bedside
  1. Both nurses need to sign the Blood Transfusion Record 
Hang Blood Product 
  1. Refer to Bloody Easy for administration and equipment needed 
Monitor Patient
  1. Vital Signs/Lung Sounds: Pre-transfusion/Start of Transfusion/15 Minute Mark/Q1H until transfusion complete/4 hours post-transfusion
  2. For Transfusion Reaction follow instructions on Blood Transfusion Record
Finish Transfusion
  1. Mark END time and sign for completion on Blood Transfusion Record
  2. Discard Blood Product in Biohazard Bin (Unless reaction occurred – keep blood product and tubing for lab to analyze) 
  3. Keep Blood Transfusion Record in Lab Section in Patient’s Chart 
  4. Obtain Vital Signs 4 hours post-transfusion
